Note: Be sure to have all members of the entity complete the Borrowing Authorization form. This includes name, signature, title and ownership percentages.

Note: Please choose the nature of authority for each member listed. It is important that we know the duration of the members authority whether it is Evergreen (always has authority), for a specified time, or is property specific for the current loan being submitted.
